Overview & Procedure

What the treatment is and how it's performed

What it is

A biostimulator is a product placed under the skin that stimulates the body to form new collagen. The skin becomes gradually firmer and thicker without much change in shape. There are several types, for example calcium hydroxylapatite (Radiesse, Calhydra), poly-L-lactic acid (Sculptra) and products that combine several substances. The practitioner chooses the product for your skin and the area.

  • Usually one to three treatments four to six weeks apart, depending on the product.
  • Maintenance after one to two years.

  1. The doctor or practitioner assesses your skin and face shape.
  2. The skin is cleaned and disinfected. Numbing cream can be applied.
  3. The product is placed with a fine needle or blunt cannula, often in a fan pattern under the skin.
  4. The area is massaged to spread the product.

  • Avoid alcohol for the last day and supplements such as fish oil for the last week. This can mean fewer bruises.
  • Come without make-up.
  • Stop using retinol and acids three days before the treatment.

The treatment itself takes 30–45 minutes.

Experience & Recovery

Pain level, downtime and healing

Most people rate the discomfort 2–4 out of 10. The product often contains a local anaesthetic, and numbing cream can be used.

  • Common: swelling, tenderness, redness and small bruises.
  • Uncommon: small lumps under the skin that can be felt but are rarely seen.
  • Rare: infection or inflammatory lumps that appear weeks or months later.
  • Very rare: the product blocks a blood vessel. This can damage the skin and, at worst, the eyesight.

  • Follow the practitioner's advice on massage. After some products you need to massage the area for several days.
  • Avoid hard exercise, heat and alcohol for the first day.
  • Avoid strong sun until swelling and bruising have gone.

Usually 1–3 days of swelling. Swelling, tenderness and small bruises usually settle within a week.

Expectations & Safety

Who it's for and safety information

  • Biostimulators work gradually. The result builds up over weeks and months as the skin forms new collagen.
  • Products with calcium hydroxylapatite or poly-L-lactic acid cannot be dissolved with an enzyme the way hyaluronic acid can.
  • Biostimulators are not placed in the lips.

  • Skin that has become thinner and laxer with age.
  • A face and neck that have lost firmness.
  • Anyone who wants a gradual, natural result.

  • Anyone who wants an immediate result.
  • A lot of loose skin. Surgery may suit better.
  • The lips and the area just under the eyes.

The result builds up over two to three months and often lasts one to two years, depending on the product.

  • Pregnancy and breastfeeding.
  • Infection, wounds, cold sores or inflammation in the area.
  • Known allergy to the contents or to local anaesthetics.
  • A tendency to raised, growing scars (keloids).
  • Bleeding disorders. If you take blood-thinning medicine, tell the clinic.
  • Active autoimmune disease or medicines that suppress the immune system, unless a doctor has cleared you.

Contact the clinic at once if you get severe or increasing pain, pale, white or bluish skin or a net-like pattern in the skin. Call 113 if your vision becomes blurred or lost. Also contact the clinic if you get a fever, increasing redness, warmth or pus.
